HOW TO COMPLETE ONE OF YOUR RECORD FORMS

MY 4-H YEAR - the green form.  You need a new one each year.

Start with placing “My 4-H Year” in the front of your folder or ring binder.  This record introduces the great, wonderful YOU!  Who are you? Are picture of you would be great.  What do you want to accomplish in 4-H this year?  What projects are you enrolled in?  As the year goes along keep filling in the other areas on the form such as what committees did you work on?  Give any talks or demonstrations?  Participate in activities?  What awards or recognition did you get?  Add extra pages as you need them.  Be creative!  This is “your” record.

¨ Page 1  -  These are my Goals: list at least three. This is a list of my projects: you need to be enrolled in at least one project.

¨ Page 2  -  Club and Project Participation - number of meetings attended - check or make notes;  Committees & Office Summary - name of committee/office with description or what you needed to do;  Promotion Presentations - promoting through newspaper, radio, displays...

¨ Page 3  -  Communication activities: (demonstrations, talks, reports, public speaking, etc.);  Awards & Honors: included 4-H and other awards

¨ Page 4  -  Use this page to record all activities, events and experiences as you have them.  Include such things as workshops, camps, school, sports, church, home chores, civic and volunteer activities.

Records are like journals they’re a way to keep track of what you do and when you do it.  Think of your records like an entry book, kept regularly.  The important point is:

When something happens, write it down.
